Ellen, Thank you for promoting this excellent project, it is through conversations in people's home environments with their families that is truly needed to help resolve this enormous health care issue. As a health care provider I see daily what happens when this conversation has not occurred. I wrote a very personal story on this topic that helps open the conversation and shows by example how this can enhance your life. The book is called Breathe by Anne Bland (pen name for Denise Baird Schwartz, MS, RD, FADA, CNSD). Royalty for the book is donated for the Millennium Development Goal #1 to eradicate extreme poverty.

If you're looking for information about having the conversation with your loved ones, go to www.honoringchoices.org. This is the continuing work of the Twin Cities Medical Society and Twin Cities Public Television.
You'll find people talking about their own end-of-life experiences both good and bad. There are documentaries with family stories and information from medical staffs and caregivers. There is also a resource page for more information.

History is an unbroken line of interconnected content from cave drawings through oral traditions and the spoken word, and finally to the written word. We know where we have come from as collective humanity. Many of us can go back several generations without having to reach for the family album. Let us for a brief moment look at an adopted child; where does her/her history begin? When that adopted child has children, their history will begin with their parents. When we remove grandma or granddad from the home, an important link with the knowledge of the past is lost or at the very least becomes blurred. What is the real cost of losing that connection? Home is a place where people live their lives, raise their children and every effort should be made to allow them to pass into the next realm from there, with dignity and grace.
